Qualifications:
- Must be a pharmacist, licensed in West Virginia
- Must be a member of West Virginia Pharmacists Association
- Must be actively practicing pharmacy
- Must have demonstrated innovative pharmacy practice resulting in improved patient care
- Cannot be a previous recipient of the Award
